Case History: 45-year-old male heavy smoker who presented with tachycardia, shortness of breath, hemoptysis, and significant weight loss. CT of the chest demonstrated a large mass in the right upper lobe, with mediastinal, hilar and sub-carinal adenopathy. His condition deteriorated due to respiratory failure and septic shock, which ultimately led to his death. Autopsy revealed a necrotic friable white-tan mass occupying the entire right upper lobe with metastasis to the left lung, para-esophageal tissue, bilateral adrenals, and hilar lymph nodes. Cytological analysis of the sputum, histology sections, and some immunohistochemical stains are shown.
